Peace Health Clinic – Brisbane Healthcare Services (“Peace Health Clinic”, “we”, “us”, “our”) is committed to protecting the privacy and confidentiality of your personal and health information. We handle personal information in accordance with the Privacy Act 1988 (Cth) and the Australian Privacy Principles (APPs), and, in relation to health information, applicable Queensland and Commonwealth health-records legislation.
What information we collect
The information we collect depends on your interaction with us and may include:
- Personal details – name, date of birth, address, phone number, email address, and emergency contact details.
- Health information (sensitive information) – your medical history, current health concerns, medications, allergies, treatment and consultation records, and other clinical information relevant to your care.
- Billing and identification details – Medicare, DVA or health-fund details and payment information where relevant.
- Website and enquiry information – information you provide through our online enquiry, booking or contact forms, and information collected automatically when you use our website (see “Our website” below).
- Online store information – where you purchase products through our website, order, delivery and payment-processing details.
Health information is “sensitive information” under the Privacy Act and is afforded a higher level of protection.
How we collect your information
Wherever practical, we collect information directly from you – in person, by telephone, by email, or through the forms on our website. In some cases, and generally with your consent, we may collect information from others involved in your care, such as a referring practitioner, specialist, pathology provider, hospital, Medicare or your health fund.
Why we collect and use your information
We collect and use your information to:
- provide you with healthcare, assessment and treatment, and manage your care;
- arrange and manage appointments, referrals and follow-up;
- process billing, payments, and Medicare, DVA or health-fund claims;
- respond to your enquiries and communicate with you;
- process and fulfil online store orders; and
- meet our legal, professional and regulatory obligations.
Disclosure of your information
We do not sell your personal or health information. We may disclose your information:
- to other health professionals and providers involved in your care (for example, specialists, pathology and imaging providers, hospitals or pharmacies), with your consent;
- to Medicare, the Department of Veterans’ Affairs or your health fund for claims and billing;
- to trusted service providers who help us operate — including Best Practice and our anaesthetic nursing software (for clinical and practice-management records), HotDoc (for online bookings and patient communications) and Tyro (for secure payment processing) — who are bound by confidentiality and only handle your information to provide their service to us; and
- where required or authorised by law, or to lessen or prevent a serious threat to life, health or safety.
Our website
When you use our website we may collect limited technical information (such as your IP address, browser type and pages visited) through cookies and similar technologies to help the site function and to understand how it is used. You can manage cookies through your browser settings. Information submitted through our enquiry, booking and contact forms is used to respond to your request. Payments made through our online store are processed securely by Tyro, our third-party payment provider; we do not store your full card details.
Storage and security
We take reasonable steps to protect your information from misuse, loss, and unauthorised access, modification or disclosure, including physical, electronic and procedural safeguards. Health records are retained for the period required by law and then securely destroyed or de-identified.
Accessing and correcting your information
You may request access to the personal and health information we hold about you, and ask us to correct it if it is inaccurate, out of date or incomplete. To make a request, please contact us using the details below. In limited circumstances permitted by law we may need to decline access, in which case we will explain why.
